Physical Therapist Assistant Home Health
Center Well Home Health is seeking a Physical Therapist Assistant for home health care. You will play a vital role in helping patients regain strength, mobility and independence—all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you will empower individuals to overcome physical limitations and improve their quality of life.
Base pay range$58,400.00/yr - $80,000.00/yr
Responsibilities- Administer the physical therapy treatment plan as developed by the Physical Therapist.
- Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities to patients.
- Accurately document daily, weekly, or monthly records of clients and submit clinical paperwork to the office on time each week.
- Attend educational meetings and coordinate/ contribute to department in-services.
- Notify the physical therapist or Executive Director of equipment failures, supply issues, staff or client changes, and other departmental concerns.
- Schedule clients and perform varied office tasks.
- Apply concepts of infection control and universal precautions in patient care activities.
- Accept clinical assignments consistent with education and competence to care for patients.
- Promote compliance with fiscal intermediaries and other third-party payors through education, coaching, and assistance as necessary.
- Associate degree in Physical Therapy from an American Physical Therapy Association.
- Current state license to practice Physical Therapy.
- Valid driver’s license, auto insurance and reliable transportation.
- Proof of current CPR certification.
- Minimum two years experience as a Physical Therapy Assistant.
- Comprehensive knowledge of changes in the geriatric population across physical, emotional, social, and biological domains.
- Self-directed, enthusiastic, and receptive to constructive feedback.
- Good verbal and written communication and organizational skills.
- Professional image/attitude for patients, families, clients, coworkers, and others.
- Maintains licensure certifications and continuing education requirements.
- Ability to work a flexible schedule and travel locally.
- Able to maintain confidentiality.
- Must read, write, and speak fluent English and maintain good attendance.
- Performs other related duties as assigned.
