The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs skilled technical care in the patient's home.

• Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team. May not alter the plan of care.
• Instructs the patient's family in care procedures and maintaining a healthy environment for the patient.
• Observes, records, and reports to the supervising nurse or physician the patient's response to treatment and any changes in condition daily.

License Requirements:

• Current LPN licensure in the state of practice
• Valid Driver's License, vehicle insurance, and access to a dependable vehicle or reliable public transportation
• CPR Certification required
