Trainee Advanced Care Practitioner

An exciting opportunity has become available to join the Acute Medicine team at PAH as a Advanced Clinical Practitioner. This position is for a qualified ACP with experience in Acute Medicine; however, a trainee position would be considered for the right candidate at Band 7 who would be expected to undertake an MSC in Advanced Practice via the apprenticeship route commencing in January 2026.
As part of a multi‑professional team led by the lead Acute Medical consultant, you will practice autonomously within an acute setting to provide patient‑centred care. Duties include assessment, examination, diagnosis and treatment within an agreed scope of practice in Acute Medicine (AAU, ED and SDEC) whilst being accountable and self‑directed in line with relevant professional conduct. You will support efficient patient‑focused service, ensuring safe referral and discharge of patients with undifferentiated and undiagnosed presentations.
Approximately 80 % of the role is clinical; 20 % involves clinical audit, teaching, self‑development, service development and research.
- Assess and prioritise patients attending or referred to acute medicine and the Same Day Emergency Care department (SDEC) with undifferentiated health/medical conditions.
- Provide a level of advanced and comprehensive assessment based on specialist knowledge.
- Direct responsibility for the management of your own case load and supervision/development of junior members of the multidisciplinary team.
- Appropriately assess, examine, investigate, diagnose and treat patients to facilitate safe management and appropriate referral or discharge.
- Contribute to the development and improvement of systems and processes that facilitate patient flow.

Qualifications- Registered healthcare professional (e.g., NMC, AHP, GPC).
- Willingness to undertake a Masters (or equivalent) degree in Advanced Practice.
- Extensive experience in the area of specialism.
- ILS/ALS.
- Evidence of continued professional development.
- ALS provider/instructor.
- Experience working clinically in the department.
- Portfolio of evidence showing achievements in practice.
- Evidence of ability to undertake audits, feedback and demonstrate outcomes.
- Experience of leading service improvement.
- Experience of mentorship or supervising and where applicable have course accreditation.
- In‑depth understanding of quality issues including clinical governance.
- 5 years post‑registration experience.
