The role:

As a Support Worker, you will provide care, encouragement, and development opportunities to our Care Conne

X, Together Conne

X and Youth Conne

X clients. Working as part of a dedicated team, you’ll deliver engaging activities and provide tailored support to people of all ages. You’ll help each person you support to build confidence, independence, and meaningful connections.

  • Providing professional care and support to vulnerable people
  • Providing person-centred care
  • Conducting client needs assessments and reviews
  • Organising play and leisure activities for children, young people and adults
  • Developing and ensuring regular review of individual care and support plans
  • Is educated to GCSE level
  • Holds a health and social care qualification
  • Is passionate about supporting vulnerable people
  • Ideally has some experience working in the health and social care sector
  • Is able to work individually and as part of a team
  • Has an outgoing, positive, empathetic personality and a clear communication style
