The London Borough of Harrow is seeking a dedicated Team Manager to lead a newly remodelled Young Adulthood (18-25) team within the Mental Health, Learning Disability and Autism service. You will guide social workers, occupational therapists and care managers to deliver person-centred, evidence-based care that supports independence and long-term planning.
You will drive the Transitions Framework, coordinate with Health, providers and advocates, and ensure safeguarding and capacity assessments
