Tobe an active member of the multidisciplinary team in developing and maintaininga ward environment that facilitates the provision of Best Practice in terms of clinical care.

To continuously manage risk assessment and management plans, ensuring a safe and therapeutic environment.

Toact as shift coordinator as allocated

Main duties of the job

Being the Named Nurse for an allocated group of patients

Maintaining a professional and high standard ofpatient care

Planning, implementing and evaluating care

To liaise and offer support to relatives and significant others

Completing all relevant documentation in accordance with Pathfinder Ashness House procedures and professional guidelines

Remaining up to date with clinical developments

Compliance with the NMC code of conduct

Ensuring patients rights to confidentiality arerespected and that Pathfinder Ashness House and professional policies are adhered to.

Adhering to all Pathfinder Ashness House policies, procedures and local protocols

To utilise resources appropriately and minimise waste

Ashness House is a 26 bedded unit for men with complex mental health needs who require rehabilitation and support with their continued recovery pathway.

We are a small, friendly MDT, who primary focus is on supporting our residents to achieve their treatment goals. We live our values every day.

Job responsibilities

1. To adhere to the named nurse concept to ensure that nursing care is organised to promote individualised approach to care

2. To ensure the completion of necessary documentation of patient care planning and all other nursing and clinical documentation.

3. In conjunction with other members of the multidisciplinary team, evaluate the effectiveness of treatment and care

4. To report concerns about patient care, treat mentor progress to senior nursing staff

5. To ensure that Care Programme Approach processis implemented

6. To ensure that there is adherence to the Mental Health Act 1983 and that patients rights are upheld at all times

7. To ensure the completion of clinical and administrative documentation in accordance with Pathfinder Ashness House procedure

8. To adhere to Pathfinder Ashness House policy and procedure and current legislation in relation to witnessing, administering and storage of medication

9. To advise non clinical staff within the hospital on management of patients with mental illness

10. To initiate/participate in nursing and other research as appropriate

Person Specification Experience
  • Ability to assess, plan, implement and evaluate patient care & act accordingly if changes occur.
  • Keeping accurate records.
  • Knowledge of medication rounds and the safe administration of drugs.
  • Practical experience of providing high quality care
Qualifications
  • Registered NMC Nurse RMN
Additional Qualities
  • Willingness to undertake further training as required.
  • Prepared to work flexible shifts.
  • Motivated to provide the highest standard of care in a changing service.
  • Awareness of Health & Safety issues
