Position PurposeOrthopedic Patient Coordinator (OPC) is responsible for planning a safe patient discharge plan by calling all arthroplasty/spine patients before day of surgery to review patient-specific post‑discharge plan and arrange for appropriate in‑home or outpatient service/therapies. Partners with physicians and office staff to meet length‑of‑stay targets (O:
E ratios) and to assure early pre‑operative involvement with patients that have medically or socially complex situations, ensuring a dual plan is in place before surgery day. Partners with community providers to ensure services are provided in a timely manner, and identify and mitigate issues that may occur between the provider and the patient.
- Partner with Surgeon, Patient and Family to have a post‑hospital therapy plan for in‑home or outpatient services well before the day of surgery.
- Notify outpatient vendor before day of surgery to assure sufficient community resources.
- Visit all patients during hospital stay to verify discharge plan and notify outpatient vendor if changes occur.
- Initiate placement for skilled nursing facility (SNF) or rehabilitation needs and make referrals for complex needs post‑hospitalization (e.g., IV therapy or wound‑vac therapy).
- Obtain insurance authorization and necessary paperwork when applicable, e.g. Medicare Observation Form, Important Message from Medicare.
- Facilitate completion of discharge paperwork including sending documentation via Allscripts to agencies.
- Provide patient experience feedback about the outpatient agency’s care to that agency and/or the surgeon.
- Associate’s degree in a related field preferred.
- Three (3) years of healthcare experience.
- Knowledge of Allscripts/EPIC.
- Knowledge of medical terminology.
- Full time 40 hrs on days.
