Senior Counselor-Intensive Family Preservation

Wheeler Health invites applications for the Senior Counselor-Intensive Family Preservation role.

Position Summary

The Senior Counselor provides direct services, case management, and coordination of care to an assigned caseload of individuals and families. The role accesses needed supports and resources in the community, facilitates ongoing communication and advocacy between the family, DCF, school, probation and other service providers. The Senior Counselor may also facilitate or co-facilitate groups in accordance to standard practice.

Compensation and Benefits

Sign‑on Bonus: $4,000 (half upon hire, half at one year of service). Bilingual stipend: $2,288 annually. Hourly rate: $21.

Education and Experience / Qualifications

BA/BS in Psychology, Social Work, Marriage and Family Therapy, Counseling or related field is required. Experience working with populations with mental health and/or co‑occurring substance use disorders is required. Effective time management, strong verbal and written communication skills are required. Valid driver’s license is required.

Essential Duties and Responsibilities
  • Develop case management plans and provide direct case management services to assigned clients and families.
  • Provide front‑line crisis support via phone, in‑home and community settings as needed.
  • Determine processes to follow based on client presentation and strengthen client strengths.
  • Offer group, individual and family counseling with appropriate supervision and coordinate treatment phases with internal and external service providers.
  • Maintain records of counseling, coordination of care and case management activities for reporting and tracking purposes.
  • Demonstrate knowledge of basic psychopathology, crisis theory and other behavioral health concerns.
  • Support daily program‑specific operational functions as required and may transport clients to appointments or linkages as appropriate.
  • Provide safety planning and crisis intervention with appropriate supervision, including rotating on‑call coverage.
  • Participate in the development, implementation and facilitation of treatment plans and timelines for strategies and next steps.
  • Make clinically appropriate referrals for identified support and service needs in a timely manner.
  • Meet direct service productivity goals as defined by program and adhere to practice standards.
  • Identify potential risk factors and take measures to reduce those risks in the home through appropriate supervision.
  • Communicate in a positive, effective manner with client, family and all internal and external providers.
Work Experience and Qualifications Highlights

Minimum 2‑3 years of direct client services experience in mental health or related field. Certified counseling license or eligibility for licensure is preferred.
