Trinity Health Of New England At Home serves patients and their loved ones by providing a variety of in-home services including nursing, social work, and physical/occupational/speech therapies. We are a Catholic, mission‑driven, non‑profit organization affiliated with Johnson Memorial Hospital of the Trinity Health Of New England health system.

Shift: Monday-Friday, 8 am to 4:30 pm, weekend/holiday rotation as required

NOTE:
This position is 100% home care providing services in patients' homes.
Provides skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse.
What You Will Do- Plan and coordinate client visits based on physician orders, client needs, and available resources.
- Administer medications and perform treatments according to care plans.
- Assess client conditions through observation and evaluation; report changes promptly and update care plans as needed.
- Educate clients and families on care procedures and health management.
- Document all care activities accurately and submit reports in a timely manner.
- Collaborate with healthcare team during care conferences and maintain effective communication with clients and families.
- Ensure compliance with federal, state, and agency regulations, policies, and ethical standards.
- Support continuous quality improvement and uphold organizational mission and values.
- Graduate from an accredited or state‑approved school of practical nursing.
- Current licensure in the state of practice.
- Must have a minimum of one (1) year experience in an acute care setting.
- Home healthcare experience preferred.
- Must possess interpersonal skills sufficient to interact effectively with clients and their families as well as all interdisciplinary caregivers, peers, subordinates and supervisors.
- Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
- Must have current driver’s license and reliable transportation to and from work site.
- Ability to consistently demonstrate the Organizational Core Competencies.
