Location:

Shift: Monday–Friday 8:00 AM – 4:30 PM; no weekends, no holidays.

Job Summary

The HVI Interventional Cardiology Nurse Navigator coordinates the individualized patient- and family‑centered care of cardiac patients throughout the care continuum. Working closely with physicians in the Cardiac Cath lab, the Navigator develops the patient’s care plan, serves as a patient advocate, educator, and liaison between the patient and all other services after discharge. The role ensures efficient transitions of care for all referred cardiac patients, initially focusing on Hartford Hospital Medical Group Interventional Cardiology elective patients admitted to JB2 with plans to extend as needed.

The Navigator also collaborates with office staff in the Medical Office Building to identify high‑risk patients—such as those with caregiver issues, compliance problems, high anxiety, or other special needs—by working closely with the Cath Lab Manager to coordinate with referring physicians and their office staff.

Qualifications

Education: Bachelor’s Degree in Nursing Required.
Master’s Degree/CNL preferred.

Experience: Experience as a registered nurse in Acute care strongly preferred. At least 2 years in cardiology Cath Lab or EP preferred.

Knowledge, Skills and Ability Requirements:

  • Must be self‑directed and able to function independently while maintaining an effective presence in a team setting.
  • Effective leadership skills with the ability to clearly communicate with tact and confidentiality.
  • Ability to adapt and be flexible to the needs of the Heart and Vascular Institute as this role evolves.
  • Ability to utilize advanced critical thinking and judgment to make safe recommendations.
