We are a community-based, nonprofit healthcare organization providing comprehensive medical, behavioral health, dental, pharmacy, and specialty services to patients throughout Hartford and Windsor. • Provide comprehensive primary and ambulatory care to a diverse patient population
• Obtain medical histories, perform assessments, diagnose conditions, and interpret clinical findings
• Develop and manage evidence-based treatment plans in accordance with established clinical standards
• Educate patients and collaborate with physicians, nurses, behavioral health professionals, and other members of the care team
• Maintain timely, accurate, and thorough clinical documentation within the electronic health record
• Support organizational quality measures and continuity-of-care initiatives

• Required:
  
  Active Connecticut APRN or Physician Assistant license
• Required:
  
  Graduate of an accredited Nurse Practitioner or Physician Assistant program
• Required:
  
  DEA and Connecticut controlled-substance registration, or eligibility to obtain both
• Preferred:
  Practice experience as a provider
