Today, we continue to achieve our mission "to build a community of strong, healthy families who protect and nurture children" by providing a full range of behavioral health, early childhood and youth development, substance use treatment and support services for children, adults and families in the Greater Hartford, Connecticut region.
Starting salary: $47,500 Bilingual: $49,500
JOB SUMMARYThe Case Manager will provide comprehensive case management services to clients and their families/significant others to ensure they have access to comprehensive, community-based services, and that all requirements from referral sources are met.
Youth Diversion TeamThe Case Manager will assist the Case Management Supervisor in all aspects of the Youth Diversion Team Program and have a Bachelor's Degree in social work, psychology, or counseling or equivalent experience. The Case Manager will maintain strong working relationships with the Police Department, Public Schools, Juvenile Probation, DCF and all area service providers who may provide services to child/youth and families appearing before the community YDT;
will have strong interpersonal skills and the ability to engage resistant child/youth and family; maintain separate record on each YDT program case, assist in maintaining the forms for the community YDT; obtain incident reports, medical, academic, social, and physical histories of child/youth referred to the community YDT and provide these materials to the community YDT members as needed; meet with each child/youth and his or her family to determine eligibility for the Youth Diversion Team Program, gain parental and child/youth agreement to participate in the Youth Diversion Team Program, ensure the signing of appropriate forms by the parent(s) and child/youth, and perform required screenings with the child/youth and prepare reports for the community YDT concerning those assessments, including a determination concerning whether more comprehensive assessments are recommended;
present individual cases to the YDT; maintain minutes of each YDT meeting, make arrangements with community resource providers to implement the community YDT recommendations; maintain ongoing family and collateral contact as needed to ensure diversion expectations are being met; write progress reports; assist in training of community YDT members and community agencies in the workings of the Youth Diversion Team Program;
ensure criminal records identifying the child/youth will be destroyed in accordance with Connecticut Statutes and perform all other related duties as required.
Travel is required throughout the state of Connecticut. Our programs serve the following towns: Bolton, Lebanon, Bozrah, Lisbon, Chaplin, North Stonington, East Windsor, Preston, Franklin, Sprague, Scotland.
KEY RESPONSIBILITIESEDUCATION
- Bachelor's degree required.
EXPERIENCE
- At least 3 years of direct experience in SUD treatment, with familiarity with CM protocols and evidence-based practices.
- Must be culturally competent to serve diverse client population; bi-lingual (English/Spanish) strongly preferred.
- Excellent oral and written skills and ability to communicate comfortably with children, parents, and both primary care and behavioral health professionals.
