The Village for Families and Children in Hartford, CT is seeking a Parent Navigator to advocate, mentor, engage, empower and support families to take the lead in their Plan of Care. Services are in-home and/or within the community.
Responsibilities include assessing family needs, assisting with access to medical coverage, mental health and schools, and connecting families to community resources; requires a high school diploma and Spanish preferred, with a valid driver license for home visits.
