Community Renewal Team (CRT) is seeking a Case Manager
- Level 1 to coordinate intake, assessments and referrals for Veterans and shelter clients in Hartford, CT. You will monitor daily contacts, develop individualized service plans and document progress in STEPS and related systems.
The role requires an Associate's degree in Human Services or related field, 3–5 years in Criminal Justice or Social Work, and HIPAA confidentiality training. Evening/weekend availability is required.
