HospiceRN role
The Hospice RN focuses on providing pain management and comfort for patients with terminal or chronic illness, and communicates and coordinates care needs with the interdisciplinary team, the patient, and families/caregivers.
The RN responsibilities- Develops a care plan in collaboration with the patient’s physician, the patient, and their family or primary caregivers upon admission. Periodically reviews and recertifies the patient care plan as appropriate.
- Coordinates the patient’s care plan through interdisciplinary communication and case conferences, as appropriate.
- Provides ongoing assessment of the patient’s clinical, functional, and emotional status; identifies problems and progress toward goals. Communicates significant changes to the patient’s physician and VNH colleagues.
- Documents evaluations, patient response, and progress toward goals.
- Teaches the patient and caregiver care techniques and end-of-life symptom management as appropriate.
- Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school of nursing.
- Medical/Surgical/Oncology nursing and/or hospice care experience preferred.
- Minimum Pay: $41.00
- Maximum Pay: $57.75
