Travel Registered Nurse – Case Management

Bring your clinical expertise and case‑management skills to Vermont’s White River Junction, leading patient transitions from hospital to home, coordinating multidisciplinary teams, and ensuring timely access to care.

• Conduct in‑depth psychosocial and medical assessments to identify care barriers.
• Develop comprehensive care plans, coordinate resources, and advocate for patients across diverse settings.
• Lead discharge planning and transitions to home, skilled nursing, or community supports.
• Collaborate with physicians, social workers, and post‑acute providers; manage utilization reviews and documentation for regulatory and payer compliance.
• Monitor patient progress, measure outcomes, and adjust plans for chronic disease management, medication reconciliation, and high‑risk follow‑up.
• Opportunities for program development, complex case oversight, or leadership within care‑coordination teams.

• Weekly pay: $2,699 – $2,837 (guaranteed 36 hrs/week)
