Licensed Nursing Assistant (LNA) – Hospice and Home Health
Base pay range: $20.68/hr – $32.05/hr

- Under the supervision of the RN and Case Manager, perform personal care services according to the individual patient’s treatment plan (e.g., simple non‑sterile dressings, bowel and bladder programs, vital sign collection, PT/INR testing).
- Assist patients in maintaining personal hygiene according to the treatment plan.
- Perform routine housekeeping tasks to promote a safe and comfortable environment (changing bed linens, completing laundry/dishes, preparing simple meals).
- Observe and report changes in the patient’s physical or mental condition to the RN and Case Manager; complete documentation in a timely and accurate manner; coordinate with other professional staff when the care plan needs revision.
- Accompany patients when they are permitted outside the home; promote mental alertness through conversation and activity; utilize infection control practices with patient care services.
- Licensed Nursing Assistant in Vermont and New Hampshire.
- High school graduate or equivalent.
- Minimum Pay: $20.68
- Maximum Pay: $32.05
