Out of Hours Care Coordinator (Hartlepool)

Shift Pattern: Role is to work every other Saturday and Sunday ONLY although optional extra daysand hours are available during the week as a Home Care Worker.

Hours: 6am to 2pm AND 2pm to 1030pm

Salary: £15.00 per hour

It is ESSENTIAL that you drive and have access to your own car for this role.

Job Description: We are seeking a dedicated and organized Out of Hours (OOH) Care Coordinator to join our team in Hartlepool. The OOH Care Coordinator will be responsible for managing and coordinating care services during weekends, ensuring that our clients receive continuous and reliable support.

Key Responsibilities:

  • Coordinate and manage care services during weekends.
  • Respond to emergency calls and provide immediate assistance as needed.
  • Schedule and allocate care staff to meet client needs.
  • Maintain accurate records of all care activities and communications.
  • Liaise with clients, families, and healthcare professionals to ensure seamless care delivery.
  • Provide support and guidance to care staff during out-of-hours shifts.

Requirements:

  • Previous experience in a care coordination or similar role.
  • Excellent organizational and communication skills.
  • Ability to work independently and make decisions under pressure.
  • Compassionate and empathetic approach to client care.
  • Proficiency in using care management software and other relevant tools.
  • Availability to work weekends.
