Position DetailsPosition: RN Visit Nurse for Home Health
Salary: $85,000–$87,000 per year, depending on experience
Service Area: Southeast Metro (Hastings, Afton, Farmington, Inver Grove Heights, Cottage Grove, South St. Paul, West St. Paul, St. Paul Park, Rosemount, Eagan, Woodbury & surrounding). Mileage reimbursement: $0.70 per mile.

- Complete an initial assessment of client and family to determine home care needs. Provide a complete physical assessment and history of current and prior illness(es).
- Initiate the plan of care and make necessary revisions as client status and needs change.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, including the client and the family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician. Provide direct client care as defined in the State Nurse Practice Act.
- Counsel the client and family in meeting nursing and related needs.
- Provide health care instructions to the client as appropriate per assessment and plan of care.
- Identify discharge planning needs as part of the care plan development and implement prior to discharge of the client.
- Act as case manager when assigned by the Clinical Supervisor and coordinate patient care for the assigned caseload.
- Lead by example in establishing rapport with clients, clinicians, partners, and the community.
- Ensure positive/professional communication within the team.
- Resolve customer service issues and raise concerns as needed.
- Prepare clinical notes and update the primary physician at least every 60 days.
- Communicate with the physician regarding the client’s needs and report any changes in the client’s condition; obtain/receive physician’s orders as required.
- Communicate with community health persons to coordinate the care plan.
- Complete all documentation related to the care plan by 10 a.m. the following day.
- Understand and comply with all rules and regulations governing the responsibilities of a registered nurse.
- Complete orientation and annual training requirements as assigned.
- Participate in Lifespark’s event management process, including initiating reports, escalating events, and investigations.
- Promote safety in the workplace through real‑time coaching and participation in safety awareness training.
- Instruct, supervise, and evaluate home health aide care every two weeks.
- Manage client supplies, ensuring an appropriate durable medical equipment provider is utilized.
- Schedule and keep member appointments.
- Organize travel efficiently and document mileage accurately within EMR.
- Other duties as assigned.
- One to two years of recent acute care experience in an institutional setting; one year of home health care experience preferred.
- Experience in customer service or direct member contact preferred.
- Experience supervising caregivers.
- Graduate of an accredited school of nursing.
- Must pass a background check as required by Lifespark policy.
- Proof of Covid‑19 vaccination or documented exemption, per CMS Vaccine Mandate.
