Te Taiwhenua o Heretaunga (TToH) is seeking a dedicated Health Improvement Practitioner to join our friendly, dynamic, and whānau‑centred team at Hauora Heretaunga.

About the Role

This is a permanent, part-time role (32 hours per week, Monday to Friday). As a Health Improvement Practitioner, you will work within an integrated primary care setting at Hauora Heretaunga, providing brief, evidence‑based interventions that support patients experiencing challenges affecting their physical or mental wellbeing, including mood, relationships, anxiety, stress, sleep issues, and adjustment to new diagnoses.

You will work collaboratively as part of both the General Practice team and the wider TToH Health Improvement and Health Coaching workforce, ensuring whānau receive responsive and accessible support.

Key Responsibilities
  • Deliver high‑quality, evidence‑based behavioural interventions across individual, group, and whānau settings
  • Apply the behavioural health consultancy model effectively within primary care
  • Navigate health and social service systems to support patient and whānau access
  • Provide interventions for a wide range of mental and physical health issues, focusing on prevention, acute care, and chronic condition support
  • Support people to identify their goals and achieve meaningful, measurable outcomes
  • Maintain clear, concise, and compliant clinical documentation within the practice’s Patient Management System
What You Bring
  • Registered health professional with a current practising certificate
  • Experience/Qualification with CBT, ACT, FACT (an advantage)
  • Comfortable working with Māori, Pasifika, youth, and applying tikanga in practice
  • Organised, tech‑savvy, and able to work both independently and as part of a team
  • Resilient, flexible, motivated, and committed to whānau wellbeing
  • Positive, can‑do attitude with a willingness to learn and grow
  • Full NZ Driver’s Licence
