POSITION SUMMARY:

The Accountable Care Organization (ACO) Case Management Nurse is responsible for reviewing patient medical records to identify preventative care measures and screenings, such as mammograms, colonoscopies, immunizations, medication adherence, and more. Once these needs are identified, the ACO nurse coordinates and schedules the appropriate preventive services and wellness visits.

This role requires flexibility, as the nurse may be called upon to assist in other areas of the clinic as needed. Duties must be performed in a manner consistent with the Mission and Values of Hattiesburg Clinic.

EDUCATION & EXPERIENCE:

  • Graduation certificate from an accredited RN school of nursing, required
  • BLS/CPR certification

QUALIFICATIONS:

  • Strong ability to maintain strict confidentiality and handle sensitive information with discretion
  • Highly organized and detail-oriented, especially in time management
  • Ability to prioritize key objectives while managing multiple tasks simultaneously
  • Excellent written and verbal communication skills
  • Effective interpersonal, negotiation, and conflict resolution abilities
  • Proficiency with Electronic Medical Records (EMR), medical management systems, and knowledge of related clinical policies and procedures

PHYSICAL REQUIREMENTS:

  • Ability to stand and walk for up to 8 hours per day
  • Ability to bend, stoop, twist, and assist patients with movement and positioning
  • Frequent keyboarding using a traditional keyboard
  • Occasional lifting, carrying, pushing, pulling, or reaching (overhead and at/below shoulder level)
  • Constant communication (speaking and hearing) in-person and via telephone
