Job Definition

Work with the CN Guidance FACT Team to deliver services to individuals who are diagnosed with severe mental illness whose needs have not been met by more traditional service delivery approaches.

Educational Requirements
  • A bachelor's degree in social work, psychology, rehabilitation counseling, or a related field.
  • Licensure with the NYS Education Department in a specific field is preferred.
Additional Requirements
  • Current New York State Driver's License.
  • Clean DMV Record.
  • Proof of Motor Vehicle Insurance.
Experience Required

Two years' experience working with the chronically mentally ill is recommended for candidates with a master’s degree; three years for candidates with a bachelor’s degree. Competency in substance abuse, family support, or vocational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment.

Duties and Responsibilities
  • Perform routine team duties and lead responsibility for integrating dual-recovery treatment, family support, or vocational/employment training with other team members.
  • Provide assessment and service planning, quality assurance and improvement processes, utilization review, incident reporting, and coordination between Forensic ACT and other systems.
  • Engage in assessments, case management, crisis intervention, and discharge planning.
  • Maintain case records and complete documentation on time and in compliance with federal, state, and agency regulations.
  • Provide direct, Telehealth, face-to-face, and/or collateral services to recipients either in the Forensic ACT office or in the community.
  • Work a 35‑hour week, including periodic pager coverage as arranged with the Program Director or Supervisor.
  • Assist recipients in developing realistic expectations from treatment.
  • Strengthen recipients’ ability to understand and utilize therapy and find resources after treatment through proper discharge planning and termination processes.
  • Exchange information with other agencies to benefit the recipient and ensure compliance with regulations and coordination of services.
  • Utilize supervision to increase practice skills, promote better services, and preserve accountability.
  • Facilitate sharing of data with individuals to support partnered decision-making and keep clients informed of progress.
  • Complete QI training during onboarding and participate in refresher training as needed.
  • Cultivate a culture of inclusion for all employees that respects strengths, views, and experiences.
  • Adhere to all safety protocols and procedures to ensure a safe working environment.
  • Maintain health and safety awareness in accordance with relevant protocols.
  • Perform other duties as assigned.
