Overview
The Occupational Therapist will be part of the Acute and Intermediate Care settings in Pembrokeshire, contributing to improvement, development and a Home First culture. Rotating at approximately 9 months, you will support the delivery of core functions in Frailty Pathway, Urgent and Emergency Care, and Acute Medical and Trauma Rehabilitation. The service emphasises collaboration with multi‑professional teams to deliver high quality, evidence based, person centred occupational therapy and continuity of care for patients.
Mainduties
- Work as part of a team to deliver an effective and efficient occupational therapy service to improve the health and wellbeing of people.
- Deliver the service in a manner appropriate to the needs of the client group, following professional codes of conduct and ethics and national/local guidelines, protocols, policies and care pathways.
- Work with colleagues or alone in peoples homes, community settings and hospitals.
- Autonomously manage an occupational therapy caseload, including those with complex presentations.
- Select and undertake assessments, develop person centred plans, deliver interventions, evaluate outcomes and adapt plans of care as needed.
- Work effectively as part of a multidisciplinary team to provide rehabilitation/recovery/reablement focused on individual needs.
- Collaborate with colleagues from Health, Local Authorities and Third Sector Organisations to achieve best outcomes.
- Support team operations, handle enquiries and undertake relevant administrative tasks.
- Support induction, education and development of others; contribute to quality improvement and defined projects.
- Appropriately delegate and supervise work of junior staff, students and volunteers as required.
Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and surrounding areas. The board employs over 13,000 staff delivering primary, community, in‑hospital, mental health and learning disabilities services, working with local authorities and voluntary sector partners.
Our Services Are Provided In- Four main hospitals:
Bronglais, Glangwili, Prince Philip, Withybush - Five community hospitals and two integrated care centres
- Numerous general practices, dental practices, pharmacies and ophthalmic services

The ability to speak Welsh is desirable;
English and/or Welsh speakers are welcome to apply. The post holder will have current HCPC registration and travel between sites to visit patients 7‑day services are developed, applicants should be prepared to work a rota over 7 days. Mileage allowance is payable and pool cars are available. A full job description and person specification are attached in supporting documents. Essential qualifications and knowledge
- Degree in Occupational Therapy
- Current HCPC Registration
- Knowledge of relevant health and social care legislation
- Knowledge of Microsoft Office software
- Understanding of rehabilitation interventions across therapies
- Understanding of clinical governance, including research and audit methodology
- Understanding and application of outcome measures to demonstrate effectiveness
Desirable:
- Knowledge of services linking across primary/secondary/intermediate care
- Experience with therapeutic equipment
- Knowledge of Health Board priorities
- Accredited Practice Educator
- Accredited Agored Assessor
Essential experience
- Significant post‑graduate experience as an occupational therapist across settings
- Managing a complex caseload
- Independent working with supervision
- Delegation and supervision of staff
- Experience handling distressing or challenging situations
- Team working and service development experience
- Experience with carers and families
- Self‑development and application of learning
- Use of adapted techniques for improving patient outcomes
Desirable experience
- Community setting experience
- Experience in the same clinical area as the post
- Experience of working with groups and using virtual platforms
