Overall Aims of the Post:

Working in partnership with other statutory, voluntary, and private sector organisations to identify carers throughout Pembrokeshire
Provide information and support to carers enabling them to better manage the impact of their caring role and enhance their wellbeing
Work with partners to ensure carers are supported to access other existing services

Areas of Responsibility

To identify new carers within Pembrokeshire and actively support new and existing carers to take up services provided by statutory and voluntary organisations e.g., Pembrokeshire County Council, welfare benefits advice, support groups, etc.
To provide a range of information, support, and assistance & outreach across Pembrokeshire to enable carers to better manage the impact of caring on their own health, financial and social situation.
* NB this will include telephone support, video support and one-to-one support within the office base, occasional home visits and/or meeting carers in community venues.
To facilitate a ‘What Matters to Me’ conversation with the carer to assess the carer’s needs for support, what the support needs are and who can deliver them.
Support carers to identify and work towards outcomes they would like to achieve.
Adopt a person‑centred, strengths‑based approach with the carer and co‑produce an action plan to address the areas of identified need.
When other services are more appropriate to meet the needs of carers, signpost and refer carers on to other agencies, organisations, and services, including social care for Community Care & Carers Needs Assessments.
To support carers to think innovatively about short breaks by ensuring that they are provided with information on taking a break, including opportunities to take time for themselves. To support carers to research and access local, regional, and national grants to which they may be eligible and may support them to continue in their caring role e.g., Carers Support Fund and Short Breaks Grants.
To support carers to consider and document contingency care plans, should the carer be unable to provide carer. This will include considering wider family and social support networks and support to register with the Carers Emergency Back‑Up Scheme.
To support the application process and increase the take up of the Carers Recognition Card by promoting the benefits to carers.
To work with partner agencies and others who may have contact with carers in their caring role, to increase the number of carers known to the service and to other services in Pembrokeshire. This may include activities such as attendance at events, manning stalls, providing briefings for professionals, attending drop‑ins, manning points of contact in surgeries, hospitals, etc.
To work with team colleagues to identify and map out existing carer resources across Pembrokeshire and highlighting any gaps in service provision to support the development of carers support services.
To raise carer awareness through involvement in carer and related community events and to be active in the planning and organisation of carer related campaigns e.g., Carers Week and Carers Rights Day.
