Job SummaryThe Transition in Place (TIP) Program is a U.S. Department of Veterans Affairs Grant and Per Diem (GPD) Program that places Veterans experiencing homelessness into apartments and works with Veterans to assume the lease within six to twelve months. This role will provide case management and supportive mental health and substance use counseling to a caseload of up to 15 Veterans.
It employs evidence‑based interventions including Motivational Interviewing, Relapse Prevention, and Trauma‑Informed Care to provide client‑centered services to Veterans.
- Maintain a caseload of up to 16 Veterans; ensure timely connection to appropriate services.
- Conduct home visits to support veterans' independent living skills for transition to permanent housing.
- Provide supportive counseling using Motivational Interviewing, Relapse Prevention, and Trauma‑Informed Care.
- Collaborate with Veterans to achieve permanent housing, including lease assumption, by increasing income, benefits, and developing self‑determination and housing stabilization skills.
- Help Veterans create realistic plans and budgets to assume the lease within 6‑12 months, accounting for all expenses.
- Complete biopsychosocial assessments, individualized service plans, and discharge plans in partnership with each Veteran.
- Coordinate with the TIP Program Manager for crisis situations and program fee collection.
- Provide community‑based referrals and appointment accompaniment during discharge planning.
- Ensure all data collection and documentation meet deadlines.
- Cooperate with VA and NECHV Human Services staff for coordinated services.
- Participate in departmental meetings, case conferences, training, and program meetings as required.
- Perform other duties as assigned.
- Bachelor’s Degree (preferred).
- 2–3 years of experience in case management with substance use disorders, mental illness, and diverse populations.
- Commitment to evidence‑based models such as Motivational Interviewing, Relapse Prevention, and Trauma‑Informed Care.
- Strong judgment and boundary‑setting.
- Crisis intervention skills.
- Strong communication and interpersonal skills.
- Massachusetts driver’s license and ability to travel throughout Merrimack Valley and to Boston.
- Comfort with home visits.
- Ability to navigate services and resources in the Merrimack Valley and surrounding communities.
- Work comfortably with a multicultural, homeless population experiencing complex challenges.
- Strong computer and electronic record‑keeping skills.
- Maintain professional and personal boundaries.
- Write clear progress notes and integrated services plans.
- Strong written and oral communication skills.
- Collaborate with other center programs and departments.
