We are seeking a dedicated and compassionate Certified Nursing Assistant (CNA) specializing in Long Term Care (LTC) to join our dynamic healthcare team. - Assist residents with daily activities such as bathing, dressing, grooming, and mobility using safe lifting practices and assistive devices like Hoyer lifts.
- Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, accurately documenting all patient information within Electronic Health Record (EHR) systems.
- Support medication administration under supervision, ensuring adherence to HIPAA regulations and maintaining patient confidentiality at all times.
- Provide specialized care for residents with dementia or Alzheimer's disease, implementing memory care techniques to promote comfort and orientation.
- Assist with medical procedures such as catheter care, phlebotomy (blood draws), and aseptic techniques during treatments to ensure infection control standards are met.
- Collaborate with nursing staff to develop personalized care plans for residents with complex needs including behavioral health, infectious disease management, or post-acute recovery.
- Ensure a safe environment by practicing safe manual handling techniques during patient transfers and responding promptly to emergency situations such as Level I Trauma Center or ICU incidents.
- Prior experience working in long-term care facilities or nursing homes is highly preferred.
- Demonstrated knowledge of medical terminology, vital signs monitoring, and basic life support (BLS) certification is required.
- Experience working with individuals with disabilities, including developmental disabilities or behavioral health conditions, is advantageous.
- Familiarity with EMR/EHR systems like Cerner or eClinical
Works enhances documentation accuracy and workflow efficiency. - Background in geriatrics or senior care settings with exposure to dementia or memory care is desirable.
- Previous hands-on experience in hospital environments such as ICU, emergency medicine, or trauma centers will be considered an asset.
- Ability to perform heavy lifting safely while maintaining proper body mechanics is essential for patient transfer methods and manual handling tasks.
This position works in a hospital, or long term care facility and is responsible for providing direct patient care through the performance of a variety of nursing support tasks in accordance with established methods and procedures. The position may work on
rotating shifts and in a variety of care units (e.g., SNF, ICF, medical, surgical, etc.). The position may also provide home care for patients.
