Position:
Case Manager II

The Case Manager II reports to the Program Coordinator and provides guidance to veterans to achieve independence. Responsibilities include assessing needs, coordinating care, facilitating intake and action plans, maintaining confidential files, conducting home visits, and supporting veterans across various programs such as transitional housing, permanent housing, workforce, SSVF, aftercare, and incarceration programs. Qualifications include a Bachelor’s Degree (preferably in Social Work, Psychology, or related fields), strong communication skills, professionalism, ability to work independently or in teams, and proficiency in Microsoft Office.

Preferred experience includes social services, working with veterans or homeless populations, and relevant certifications.
