As a crucial member of the UHN Reactivation Care Centre, you will plan, manage, and evaluate nursing care according to standards. Your expertise will guide you in participating in care planning forums and mentoring RPN students. This position requires a valid Ontario registration and a diploma in Practical Nursing, along with effective problem-solving skills.

Key Responsibilities:

• Plan and evaluate nursing care for patients and families

• Implement care based on current theoretical knowledge

• Participate in inter-professional team forums

• Provide mentorship to RPN students and staff

• Comply with hospital policies and patient safety responsibilities

Requirements:

• Valid registration with the College of Nurses of Ontario

• Diploma in Practical Nursing

• Recent clinical experience preferred

• Commitment to professional development

• Computer literacy in relevant software
