Community Nurse - Hayle Community Nursing Team

Closing date: 27 July 2026

This post is based within the Hayle Community Nursing team covering the areas of Hayle, St Ives and Marazion. We are looking for a caring, motivated and dynamic community nurse to join our large, busy, professional team. We cover several GP practices in the Penwith PCN areas, with an expectation to work across the West locality when required due to service demand.

Your role will require close collaboration with GP’s, health and social care colleagues including therapists and voluntary organisations. We would like you to possess commitment to team working and embrace innovative delivery of excellent standards of care, have a real enthusiasm for nursing in the community and a desire for ongoing professional development. You should also have an understanding of the changing climate of care delivery in the community.

The ability to travel independently across the West Locality to support other teams and to fulfil the requirements of the post is essential.

We work a 2 shift model covering the core hours 8am – 10pm including bank holidays and weekends on a rotational basis.

Main duties of the job

Following an initial induction and ongoing clinical supervision, you will be responsible for assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment. The aim is to enable them to enjoy maximum independence and optimum quality of life. To work in partnership with patients and carers to offer support and advice and promote informed choice.

To support the role of the Community Nurse Team leader. To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures and guidelines. To support all locality teams by working in a flexible way: moving to work with the team in most need on a daily, weekly or monthly basis as service provision demands.
