Cornwall Council is recruiting a Social Care Practitioner for the Penwith East PCN team, covering St Ives, Carbis Bay and Hayle. You will work with social workers to deliver high-quality assessments and care planning for adults with health and social care needs.
Role requires collaboration across teams and partners, safeguarding duties, and liaising with service users to promote independence and wellbeing. Core hours apply Monday to Friday, 8.45–17.15.
