1:00pm to 6:00pm

• Provide pharmaceutical care and medication management for skilled nursing patients.
• Review and verify medication orders for accuracy and appropriateness.
• Consult with healthcare professionals about medication therapy and patient outcomes.
• Ensure compliance with state and federal pharmacy regulations.
• Participate in interdisciplinary team meetings to optimize patient care.
• Educate nursing staff and patients on medication usage and safety.
• Maintain accurate and up-to-date patient medication records.
• Assist in training and mentoring pharmacy technicians and interns.

• Pharm
  
  D degree from an accredited pharmacy school.
• Active California pharmacist license with no disciplinary actions.
• Minimum of 2 years of experience in a clinical pharmacy setting.
• Strong knowledge of pharmacotherapy and medication management.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Detail-oriented with strong organizational skills.
• Commitment to continuous professional development and learning.
