St. Rose Hospital is seeking a dedicated Registered Nurse for our Skilled-Subacute unit. The successful candidate will provide direct patient care and collaborate effectively with physicians, the Nurse Director, and other team members to ensure the highest quality of care.
Job SummaryThe Registered Nurse will deliver hands‑on care in the Skilled-Subacute unit, communicating changes in patient condition, coordinating with the medical team, and responding rapidly to clinical needs.
Key Responsibilities- Consistently use two patient identifiers when drawing laboratory specimens, administering medications, and performing procedures.
- Avoid unacceptable abbreviations in documentation and read back all telephone orders.
- Adhere to medication reconciliation policy throughout all shifts.
- Maintain crash cart readiness; check defibrillation function and replace items after each code, as documented on the crash cart checklist.
- Comply with infection control standards (OSHA, CDC) and use appropriate personal protective equipment, including gloves, gowns, masks, and goggles, along with proper hand‑washing techniques.
- Provide a thorough, efficient change‑of‑shift handover to ensure continuity of care.
- Assist physicians with special tests and procedures; use time‑out for all bedside procedures.
- Prevent injury to self and patients by applying body‑mechanics principles and maintaining a safe environment.
- Perform pain assessment and reassessment according to the prescribed time frames for pain medication administration.
- Document medication administration on the MAR, narcotic sheet, and Pyxis systems.
- Recognize signs of abuse and report appropriately to Adult Protective Services.
- Initiate discharge and patient/family education planning, documenting in Medi‑Tech.
- Prioritize patient care based on essential needs, available resources, and time constraints.
- Develop, maintain, and update a goal‑directed plan of care based on patient outcomes.
- Perform additional duties as assigned or required.

Required Qualifications- Current and valid Registered Nurse license issued by the State of California.
- BCLS certification upon hire and maintained in good standing.
- Previous six months of experience in an Acute Care hospital or Long‑Term Care setting.
