We are now hiring a Social Service Designee to be the newest member of our team at Hayward Post Acute

Salary Range: $19.00–$20.00 per hour

Responsibilities

Under the direction of the Social Services Director, the Social Services Designee will assist each resident and family in adjusting to placement, illness, and the plan of care, helping them attain the highest practicable level of functioning in accordance with current federal and state regulations and our policies and procedures. The Designee will:

  • Assist residents and families with wishes regarding treatment and help with their transition and adjustment to the facility.
  • Participate in resident and family council meetings as requested.
  • Support Admissions and assist with discharge planning services.
Qualifications – Must Have
  • High school diploma or equivalent.
  • Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals.
  • Ability to organize, prioritize, and meet deadlines.
  • Ability to maintain confidentiality and be discreet.
  • Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long‑term care.
Qualifications – Nice to Have
  • Experience with geriatric population.
  • AA or Bachelor’s degree in a related field.
  • Creative problem‑solving skills.
  • Strong communication skills.
  • Proficient computer skills to manage records and documentation.
