We are seeking a salaried GP to cover maternity leave for four sessions per week - ideally Wednesday and Thursday. We are a forward thinking, well organised training practice with a good reputation for quality and high standards. Mainduties of the job
Key responsibilities and duties
Surgery consultations (telephone and F2F), triaging calls/online consultations as appropriate, occasional home visiting and care home cover, duty doctor sessions and administration associated with patient care
Work as a team member with all relevant providers of primary and community care services within the practice
Ensure appropriate records are maintained at all times regarding patients care and that relevant team members are kept fully informed regarding patients seen on behalf of the practice
Ensure that the practice achieves the highest possible QOF outcomes
Developing care plans for health in consultation with patients and in line with current practice disease management protocols
Support the proactive care agenda by participating in care planning and multi-disciplinary meetings
Attend team and healthcare meetings when required
Other responsibilities in the practice
Maintain confidentiality at all times
Work in accordance with and comply with all practice policies and procedures/guidelines. e.g. health and safety, prescribing, GDPR, and assist in their development
Fulfil any monitoring requirements attached to the post as required
Maintain and develop individual skills and knowledge, ensuring statutory, mandatory and Trust required training updates are completed in a timely manner.
Attend training and events organised by the practice or external agencies where appropriate
Actively participate in clinical governance within the practice

Days -To fit in with clinical needs of the practice and negotiated with above party
Annual leave -Entitlement will be 7-week pro-rata
Study leave -Entitlement will be 1-week pro-rata (£250 study budget available)
General Day to Day Structure
Morning surgery 8.30am to 12.30 (times flexible for equivalent number of patients) with admin break of 30 minute mid-morning and additional team huddle, surgery comprised of between 12-15 patients, mix of telephone andF2F. Telephone apps 10mins, F2F 15mins.
Home visits To be shared with doctors present in the surgery thatday as required and deemed medically necessary for ongoing patient care. Onaverage one per day (although may be more related to day to day needs) thiswill approximate 30 mins clinical work.
Afternoon surgery 3pm to 5.30pm (times flexible for equivalent number of patients) comprised of on average 12 appointments, again mix of telephone and F2F.
Administration time (between clinical sessions) Equating to 1.5hours for various task including but not limited to referrals, reviewing letters and blood tests, insurance form etc.
Duty doctor There will be an expectation that you will be required to cover duty doctor sessions on a rotational basis shared out proportionally to the amount of session worked/week (usually one session/week). A mixture of telephone and face-to-face appointments for urgent, on the day problems, after being triaged by the paramedic, who works with you. We aim tocap the number of contacts - AM (15) and PM (12) and the reception aim toroll-over cases after 5.30pm onto the next day.
Obviously professional responsibility remains over the duration of your shift and if clinically appropriate care for additional patients may be required. You are expected tobe present as Duty Doctor until 6.30pm on an afternoon shift.
Triage doctor We are not a triage first surgery but are managinge
Consults on a shared rotational basis between all the GPs. This is clearly an evolving area but at present there is a 1-hour slot for a doctor AM and PM toreview the undetermined eConsults. Urgent matters are dealt with by the paramedic team, purely administrative tasks are managed by the reception teamand uncertain/intermediate/routine requests are reviewed by the triage doctor.
The aim of this is to triage the request rather than to be managing the case,although this may be possible with any left over time. At present these are amounting to approximately 15 per session.
Patient list All patients register with the practice, however,each GP will have an individual allocation of patients.
Caseload
Thepost holder will manage a caseload of patients assigned to them to ensure continuity of care (although each patient has the right to be seen by his/her preferred doctor…
