Overview
The Pharmacy Resident is responsible for providing quality pharmaceutical care and services to patients and health care professionals at any ARH or affiliated unit as assigned. While the Pharmacy Resident is a registered pharmacist, they work under the guidance and tutelage of the Residency Coordinator and Director of Pharmacy in all job activities.
Responsibilities- Maintains and monitors clinical pharmacy services (such as formulary compliance, therapeutic interchange programs, intravenous-to-oral switch services, antibiotic monitoring services, etc.) as directed by the Director of Pharmacy and/or the Clinical Pharmacy Coordinator.
- Conducts appropriate clinical interventions to maximize our patients’ pharmaceutical care and accurately documents those interventions.
- Supervises and performs the functions involved in the hospital and retail drug distribution programs.
- Supervises and performs the functions involved in the I.V. Admixture Program to include the accurate and efficient preparation and delivery of total parenteral nutrition solutions, chemotherapy, antibiotics, and other medications for parenteral administration.
- Reviews and verifies medication orders for accuracy and completeness.
- Monitors patient's drug therapy to detect and/or prevent drug interactions, contraindications, and other adverse reactions.
- Uses and is proficient in the operation of the pharmacy computer system.
- Compounds and dispenses medications and other pharmaceutical products.
- Obtains pertinent patient medication and medical information.
- Provides current and accurate drug information to patients and health care professionals.
- Communicates effectively with patients and health care professionals.
- Supervises pharmacy supportive personnel.
- Serves on hospital committees as assigned.
- Performs other related duties as assigned.
- Doctor of Pharmacy (Pharm
D) is required. - Licensure eligibility in state of employment.
Must possess the physical ability needed to exert 20 to 50 pounds of force occasionally; and/or 10 to 25 pounds of force frequently; and/or greater than negligible up to 10 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for light work.
Specific Accountabilities- Responsible for properly assessing patients' pharmaceutical needs and providing pharmaceutical care.
- Responsible for the accurate and proper compounding and dispensing of prescribed medications and solutions.
- Responsible for properly counseling patients/patients' families, and appropriate health care professionals regarding drug therapy.
