REQUIRED:

Bachelor’s degree

SPECIAL

SKILLS:

Preferred experience with the courts, DJJ, DCBS, and Transitional Age Youth ages 14-25. Strong Communication and networking skills, strong written and verbal skills, some public speaking and must be willing to travel.

JOB DUTIES:
  • Engage targeted populations and assure referrals flow with minimal barriers.
  • Conduct outreach to individuals, link clients with necessary services.
  • Provide face‑to‑face in‑home and community contact as needed.
  • Ensures transportation needs are met as necessary to meet treatment needs.
  • Manage day‑to‑day collaboration between Project Partners and participate with process improvement activities as directed by program needs.
  • Meet the minimum direct time requirement of at least 10 hours weekly and completes a written, comprehensive needs assessment for identified consumers.
  • Maintains face‑to‑face in‑home/community contact.
  • Provides assistance in the development of the consumer’s treatment plan.
  • Assists the consumer in accessing services from other agencies and organizations such as DJJ,DCBS, court, community and civic groups.
  • Assists consumers in establishing and maintaining relationships required in daily living.
  • Monitors and coordinates consumer’s progress.
  • Provides case consultation as required.
  • Provides crisis assistance.
  • Performs advocacy activities on behalf of the consumer.
  • Encourages independent living.
  • Ensures transportation needs are met as necessary to meet treatment needs.
  • Participate with process improvement activities as directed by program needs.
  • Establishes and maintains client records, documenting contacts, services needed, client’s progress and other information as required.
  • Complete service documentation and billing entries within 24 hours ofservice delivery.
  • Obtain required continuing education hours and other training as necessary to meet state, agency, and credentialing requirements.
