This full-time remote role is for a Case Management Nurse. The Case Management Nurse will be responsible for coordinating care plans, overseeing medical treatments, arranging discharge planning, and ensuring efficient utilization of healthcare resources. The role involves working with patients and their families to support their medical needs, collaborating with healthcare providers, and advocating for patients’ best interests to ensure optimal care outcomes.

• Strong expertise in Nursing and Medicine, with a solid understanding of patient care principles and protocols
• Proficiency in Case Management and Discharge Planning processes to coordinate and execute care plans effectively
• Knowledge of Utilization Management to ensure efficient use of healthcare services without compromising quality
• Excellent communication and interpersonal skills to collaborate with patients, families, and multi-disciplinary teams
• Problem-solving abilities, attention to detail, and organizational skills to manage multiple cases effectively
• Current RN license and familiarity with home health or hospice care is required
• A BSN degree or a related healthcare qualification is preferred
• Proficiency with healthcare systems and documentation tools is advantageous
