Registered Nurse Case Manager - Hospice

Hospice Home Care is seeking a passionate, dedicated Hospice RN to join our team in Searcy, AR. Our hospice RNs provide expert, patient-centered care at a critical time in people’s lives. • Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient s wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order
