Esdégé-Reigersdaal zoekt een ambulant begeleider/gezinsondersteuner. Je werkt met gezinnen en cliënten met lichte verstandelijke beperking en psychiatrische problematiek in een ambulante setting, biedt oplossingsgerichte ondersteuning en coördineert zorg met diverse professionals.
Je beschikt over mbo+/hbo-niveau, BIG of SKJ registratie (of bereidheid om te registreren) en hebt rijbewijs
B. Je werkt zelfstandig, maar Teamwerk is ook essentieel voor de best passende ondersteuning.
