Medical Social Worker (MSW)

We are seeking a dedicated Medical Social Worker (MSW) to join our hospice team. The ideal candidate will have a strong background in clinical social work and a commitment to providing compassionate care to individuals of all ages, with a majority of cases involving clients over the age of 65.

• Conduct comprehensive biopsychosocial assessments for patients and their families.
• Provide support and counseling to patients and caregivers.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Assist patients in accessing appropriate community resources and services.
• Manage case documentation and maintain accurate records using the Epic EMR system.

• Master's degree in Social Work (MSW) from an accredited program.
• Minimum of 2 years of experience in clinical social work, emergency department, or medical social work.
• Active driver's license and Basic Life Support (BLS) certification.
• Must be currently pursuing or in possession of a clinical licensure (LCSW) or working towards it.
• Able to lift up to 50 lbs intermittently.
• Must have a reliable vehicle capable of navigating different driving conditions, including gravel roads and inclement weather.

This position is designed for daytime hours from 8 AM to 4:30 PM, with no on-call requirements. The successful candidate will be part of a collaborative department with support from a lead social worker.
