Care Management LPN

Care Management LPN – an integral role at Shodair Children’s Hospital, responsible for coordinating patient admissions, discharges, and transfers across our Acute and Residential units. This position ensures seamless transitions of care, supports patient and family education, and upholds the highest standards of safety and compassion.

• Admit and discharge patients from community settings and coordinate intrahospital transfers.
• Complete documentation related to admission, discharge assessments, and initial treatment plans.
• Assist units with patient arrivals, departures, therapeutic passes, and discharges.
• Provide nursing care as needed, including medication administration, patient monitoring, and crisis response support.
• Participate in treatment team meetings and patient/family education.
• Communicate effectively with multidisciplinary teams to support safe, efficient, and therapeutic care.
• Maintain compliance with hospital policies, nursing standards, and the Montana Nurse Practice Act.

The ideal candidate is a compassionate, adaptable LPN with strong clinical and organizational skills, capable of communicating effectively across disciplines, staying calm under pressure, and applying trauma‑informed care to support patients through transitions.

• Graduate of an accredited LPN program.
• Current LPN license in the State of Montana (or a Nurse Licensure Compact state).
• CPR and Nonviolent Crisis Intervention with Advanced Physical Skills certification within 3 months of hire.
• Preferred: 1–2 years of experience in psychiatric or pediatric nursing.
