NHS Greater Glasgow and Clyde (NHSGGC) is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across a wide range of clinical and non‑clinical professions and roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, and to a wider population of 2.2 million when regional and national services are included.

The Role:

NHSGGC (2016) Community Mental Health Team (CMHT) Operational Framework defines the CMHT as forming part of a whole system approach to mental health services for the adult and older adult population. The service is delivered in conjunction with Primary Care Mental Health Teams (PCMHT);
Acute Services (Crisis / Home Treatment Services & out of hours (OOH) Services); specialist Mental Health services and a range of statutory and non‑statutory services that support the delivery of care.

  • Work with people with complex mental health problems and associated risks who typically require longer‑term care and treatment, multidisciplinary engagement, and with the capacity to provide assertive care and to manage higher levels of risk.
  • Work in partnership with patients, families and carers, primary care and other agencies to design, implement and oversee comprehensive packages of health and social care where needed, to support people with complex mental health needs.
  • Deliver services in a suitable environment that is accessible to the individual (and their carers). This will include team base, GP surgery, the patients’ home or other appropriate community setting.

Interventions delivered by the CMHTs will be on a “stepped / matched care” basis, based on individualised assessment of need, and will be governed by the principle of the delivery of the minimum effective level of intervention.

All patients being cared for have a Named Nurse and/or Keyworker who is tasked with working closely with their identified patients/carers to develop a participative, therapeutic relationship and deliver high quality mental health care in an environment where patients feel valued and where confidentiality is respected.

Duration, Location, and Working Pattern:
  • Permanent post
  • Full time 37 hours per week
  • Shift Pattern – Monday – Thursday 9 am‑5 pm, Friday 9 am‑4:30 pm

Key Responsibilities:
  • Clinical Leadership & Care Coordination:
    Acting as Named Nurse/Practitioner for a complex caseload, completing holistic assessments, managing risk, and planning, implementing, and evaluating specialist care.
  • Therapeutic Practice:
    Ensuring care is delivered in the least restrictive way, supporting staff to respond to issues such as vulnerability, domestic abuse, safeguarding, and self‑harm, and promoting therapeutic, respectful, culturally sensitive relationships.
  • Professional Standards & Evidence‑Based Practice:
    Ensuring nursing practice aligns with NMC standards and relevant legislation, and that staff deliver evidence‑based psychological therapies and person‑centred care.
  • Governance & Quality Assurance:
    Leading on clinical governance, maintaining effective communication, policies, audits, learning culture, research engagement, and risk‑reduction strategies.
  • Supervision & Staff Development:
    Providing line management and clinical supervision to Band 6 nurses, monitoring performance, and ensuring ongoing professional development.
  • Service Coordination & Partnership Working:
    Supporting safe transitions of care, liaising with referrers and partner agencies, and contributing to service development and quality learning environments.
  • Operational Responsibilities:
    Managing delegated budgets, producing formal reports, contributing to team performance indicators, and supporting the service manager with team development and operational issues.
