Overview
We seek an individual who will support the Team Leader in the development and implementation of a multi-professional Crisis Resolution/Home Treatment Team providing a high-quality service to meet the needs of people in acute phases of mental illness, and their carers. This role will cover the in-reach service to support the Acute Assessment units based at Kingfisher Court in Radlett. The role will be based in Hemel Hempstead.
You must hold a full valid driving licence and have access to a car to use regularly for business purposes (unless you have a disability as defined by the Equality Act 2010).

Responsibilities- Provide urgent assessments to those considered to have acute mental health concerns.
- Develop robust home treatment options as an alternative to hospital admission.
- Provide leadership and supervision within the multi-disciplinary staff group as appropriate.
- Act as gatekeeper for hospital admission.
- Positively manage risk in the acute care pathway, whilst enabling and empowering an individual's recovery journey.
- Support the Team Leader in establishing and maintaining effective inter-team and inter-agency communication.
- Provide effective liaison/communication relating to referral and admission to, and discharge from the CRHT Team across in-patient and community service.
- Maintain effective joint working with Sector ACMHSs, PATH and other community-based teams according to local circumstances.
- Ensure that all relevant information regarding client care is communicated to the appropriate persons within and outside the team, bringing to the CRHT Team Manager's attention issues of concern regarding practice or workload.
- Ensure effective communications with relatives, visitors, voluntary organisations and the general public, encouraging participation in care delivery.
- Respond sensitively to the needs of relatives and carers, often in a highly emotive setting.
- Liaise with other professionals in relation to client care, actively participating in care planning and review meetings, and attending case conferences as appropriate.
- Advise others as to the role and practices of the CRHTT.
- Maintain positive working relationships and clear communications with team members and other professionals.
- Post-registration experience with at least some experience at band 6.
- Experience in acute and community areas.
- Evidence of continuing professional development.
- Experience in care planning, change management and use of Microsoft Office.
- Knowledge of physical health contributions to mental health issues, difficulties and assessment.
- Appropriate UK professional registration.
- Desirable criteria:
Experience in working with acute mental health problems in a community setting, RMN or Dip / BSc (Hons) in Mental Health Nursing. - Desirable criteria: ENB 998 / Preceptorship and Mentorship.
- Desirable criteria: ENB 812 / BSc (Hons) Specialist Community Nursing Practice (Mental Health) or similar training.
- An understanding of the needs of people with acute mental health problems.
- Ability to communicate effectively within a multiprofessional team.
- Confidence to work autonomously in the community and use own initiative to make decisions in complex situations.
- Ability to provide and receive complex, sensitive or contentious information, where persuasive, motivational, negotiating, training, empathic or reassuring skills are required.
