Hertfordshire County Council is seeking a Preparing for Adulthood Practitioner to support young people aged 14 to 25 and their families. You will manage a caseload, assess needs under relevant legislation, and coordinate with schools, health professionals and SEND teams.
The role requires a proactive, well-organised approach and the ability to work under pressure. Starting salary is £34,434 with progression to £37,280 per annum (pro rata for part time).
