Valley Health System, a leading integrated health network serving more than two million people in Southern Nevada, seeks an RN Case Manager for our West Henderson Hospital to provide high‑quality discharge planning and utilization management services.

• Establish safe, individualized discharge plans and facilitate timely utilization management throughout the care continuum.
• Collaborate proactively with patients, families, and the healthcare team to ensure optimal care transitions and achieve maximum reimbursement for all patient visits.
• Provide competency‑based case management for the assigned unit/department, meeting both clinical and social needs of patients at discharge.

• Education: Graduate of an accredited school of nursing.
• Experience: Minimum three years in varied clinical settings; preferred two years in Utilization Review, Utilization Management, or Case Management.
• Knowledge: Understanding of social and physical factors affecting functional status at discharge and familiarity with community resources to meet post‑discharge needs.

  Computer proficiency including word processing, spreadsheets, and data collection/management programs.
• License/Certification: Current Nevada RN license.
• Other: Demonstrated ability to provide age‑appropriate care/service for patients in the assigned unit/department.
