Telos Health Systems staffs on a pay per encounter basis:
Psychotherapists, Psychologists, PsyD or Ph. D, Licensed Clinical Social Workers (LCSW), Licensed Clinical Mental Health Counselors (LCMHC) and Licensed Marriage and Family Therapists (LMFT) in Skilled Nursing Facilities and Assisted Living Facilities to bring quality mental health services to those communities.

• Perform comprehensive diagnostic assessments of your patients’ needs and psychosocial status.
• Develop a comprehensive, individualized treatment plan and goals for your patients.
• Provide psychotherapy, group, and family therapy if applicable.
• Collaborate with members of the facility’s interdisciplinary team to develop treatment goals for the patient.
• Provide appropriate documentation of all pertinent information obtained in interviews, consultation with other healthcare professionals, referrals, contacts, and supportive services provided to each patient.
• Complete necessary documentation and reporting in accordance with departmental standards and Federal and State regulations.

Minimum commitment 2 days a week – out of Monday‑Friday, you pick the days – daytime hours! Pay: W2, Pay per encounter, fee for service role.

• Licensure / Certification:
  Psychotherapists, Psychologists, PsyD or Ph.D., Licensed Clinical Social Workers (LCSW), Licensed Clinical Mental Health Counselors (LCMHC) and Licensed Marriage and Family Therapists (LMFT) (specific to the state).
• Education:
  
  Master’s degree required.
