About the Role

You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include:

* Providing comprehensive primary care services to a manageable patient panel of 12-20 patients per day

* Collaborating with interdisciplinary teams to optimize patient outcomes

* Ensuring smooth transitions of care with hospice, palliative care, and home care agencies

* Maintaining accurate and timely documentation in our EMR system (Gehrimed)

Qualifications

* Graduate degree as a nurse practitioner or physician assistant

* Active and unrestricted NP or PA license in North Carolina

* Strong clinical assessment and diagnostic skills

* Excellent communication and interpersonal abilities

* Experience in post-acute care preferred, but not required (training available)

Compensation is fee for service (FFS). Expected compensation is estimated range of $103k to $122k annually with no cap on productivity income potential. This full-time role is benefit eligible.
