Eventus Whole Health delivers an integrated model of care to adults residing in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.
Essential Duties and Responsibilities:Provide primary-care services in accordance with Eventus Whole Health site-specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine related to long-term care facilities.
Quality/Compliance- Review and sign MARs as requested by facilities, sign all orders specific for Primary Care while at the facility and/or within 24 hours if faxed or emailed, including phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders, and other relevant orders. Review and address labs within 24 hours of receipt. Manage narcotic prescriptions specific to your facility’s patients, adhering to Eventus Whole Health Narcotic Rx Policy.
Allocate sufficient time during visits for administrative paperwork. - Complete documentation and submit billing records according to policies. Provide the facility with a hard copy of electronic documentation within 48 hours for inclusion in the patient’s medical record.
- Maintain open communication with facility staff and administration. Provide verbal and written instructions or feedback regarding medications and caregiver information. Meet with the facility care coordinator or administrator before and after daily patient visits.
- Adhere to Eventus Whole Health standards, policies, procedures, and all legal, regulatory, and ethical guidelines governing healthcare delivery.
- Maintain patient confidentiality at all times, including appropriate use of cell phones, emails, text messaging, patient charts, and EMR, in compliance with HIPAA regulations.
- Consult with the clinical supervisor before refusing a resident, family, or facility request regarding resident care.
Education:
- Master's degree in Nursing from an accredited institution and a current, valid license to practice medicine issued by your state Medical Board. All other required certifications must be current and valid.

- Graduated from an accredited physician assistant program, passed the national certification exam of the National Commission on Certification of Physician Assistants, and licensed as a Physician Assistant in your state. Specialization in geriatric, family, or psychiatry is preferred.
