Tri Star Hendersonville Medical Center is seeking a Social Worker (MSW) to perform non-clinical biopsychosocial assessments and interventions across inpatient floors and the ED. You will provide crisis intervention, patient/family support, and referrals for financial or resource needs, while assisting with post-acute placement and discharge planning.
Responsibilities include collaborating with RN case managers, physicians, and insurers to identify discharge barriers and coordinate care.
