Home Health Registered Nurse; RN Hephzibah, Georgia

An experienced and compassionate Registered Nurse is needed for a rewarding home health role. This full-time contract position focuses on delivering patient-centered, skilled nursing care in a home-based setting. Youll work autonomously while enjoying support from an interdisciplinary healthcare team, ensuring your patients receive the highest quality of care.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify clinical changes, and communicate updates to care teams and physicians.
• Educate patients and families about disease processes, treatments, and preventive strategies.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides to ensure coordinated care.
• Participate in case conferences, staff meetings, and continuing education.
• Adhere to regulatory and agency standards for documentation and care delivery.
• Daily travel to patient residences within your assigned territory (mileage reimbursement provided).

Qualifications:

• Active RN license (in good standing).
• Minimum of 1 year recent home health experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Excellent clinical assessment, critical thinking, and organizational skills.
• Reliable transportation, valid drivers license, and auto insurance.
• BSN preferred; previous experience managing 2532 points per week and knowledge of Medicare/home health compliance standards are a plus.

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
